It is an integrated system of intensive medical and behavioral care navigation. Through Health Guardians, Catholic Charities offers an integrated system of intensive medical and behavioral care management and wraparound social services for this high-need patient population.<\/p>\n<p>Health Guardians currently serves St. Bernard, Plaquemines, Jefferson and Orleans Parishes. Support and assistance includes:<\/p>\n<ul>\n<li data-aria-level=\"1\" data-aria-posinset=\"1\" data-listid=\"1\">Intensive care management<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"2\" data-listid=\"1\">Transportation assistance for those otherwise unable to attend Primary Care appointments<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"3\" data-listid=\"1\">Facilitating with Health Care Coverage (Medicaid, Medicare, GNOCHC)<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"4\" data-listid=\"1\">Serving as a liaison between doctors and patients<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"5\" data-listid=\"1\">Counseling and medication management<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"6\" data-listid=\"1\">Food Assistance<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"7\" data-listid=\"1\">Enrollment in primary care medical home<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"8\" data-listid=\"1\">Prescription Assistance<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"9\" data-listid=\"1\">Application Assistance<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"10\" data-listid=\"1\">Hygiene Needs<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"11\" data-listid=\"1\">Referrals for substance abuse treatment<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"12\" data-listid=\"1\">Workforce Development Referrals<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"13\" data-listid=\"1\">Transitional housing<\/li>\n<li data-aria-level=\"1\" data-aria-posinset=\"14\" data-listid=\"1\">Health education and literacy<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Published in our 2016 Annual Report.
